This form needs to be completed within 10 days of the chapter completing any community service or philanthropy event. In addition to this report please remember that you will also need to submit the proof of completion to your council advisor.

Community Service and Philanthropy Report Form
This form is to be submitted by first of each month by the chapter president. For example on January 1 the chapter president should be submitting a complete report for all of the events that occurred during the month of December. If there is not enough on this form for all of the events that occurred in the given month please submit an additional form with those events.

The Fraternity and Sorority Life Office Conference Table can be reserved for meetings for groups of 10 or fewer people only. When reserving this space please note that no food or drink is allowed.
